On window it should be something like http://127.0.0.1:<port#>/jschtml/<formname>.htm,  not sure about Mac.
You will get the right URL once you run the form from the format browser using "Test Form".

Do you also know that URL that one types to test forms with just Omnis.. no app server setup yet.. but just Omnis can serve.. I did it before.. it’s in the manual but I have to find it.. it’s an URL just like the one you just gave.. it’s very similar anyways.. I remember that.. and it works with just the Dev copy of Omnis..
